Eligibility
- Do not qualify for assistance through Medicaid
- Do not qualify for Veterans Administration
- Currently do not have an existing health care coverage
- Located in Brooke, Ohio, Marshall, Wetzel and Tyler Counties in West Virginia or Belmont County, Ohio.
Necessary Forms
To receive health care services, patients must provide proof of identity:
- Photo ID
and proof of income, including one or more of the following, depending on the patient’s situation:
- Form 1040 or 1040A (Tax Return)
- Form 4506-T (if you don’t file incomes taxes)
- 1099 Benefits Statement (if you are on Social Security)
- W-2 from Employer
- Pay Stubs for One Month
